Millennium Eye Care provides a full range of eye care services in six convenient locations in central New Jersey. Our skilled staff of ophthalmologists, optometrists & opticians offers a team approach that provides our patients with comprehensive ophthalmic care. Dating back over 40 years, Eye Physicians & Surgeons, P.A., grew to become one of the largest groups of ophthalmologists in the state of New Jersey. In 1998, Millennium Eye Care, L.L.C. emerged, combining the subspecialty skills of a large group of Board-Certified ophthalmologists with an experienced staff of optometrists and opticians. Our patients now have access to a full range of services, including routine vision and diabetic exams, eye exams, contact lenses, treatment of glaucoma, retinal and corneal disorders, as well as cataract and oculoplastic surgery. We offer a wide selection of stylish eyeglasses. Patients in need of eye surgery will find Freehold Surgical Center, our affiliated ambulatory surgery center located adjacent to our main office in Freehold Township our convenient, close to home and patient friendly. In addition, our ophthalmologists are members of the medical staff of CentraState Medical Center in Freehold. Our goal is to match your needs with the appropriate eye doctor. With the assistance of our staff, you can choose an eye doctor from our skilled ophthalmologists and optometrists to perform your eye exam.

At Millennium Eye Care we strive to provide you and your family with the best possible patient experience. Please arrive to the office a few minutes early so that our staff can meet you and properly gather all necessary information.
What to Bring to Your Appointment
- Please arrive at least 15 minutes prior to your appointment time.
- Please bring your insurance card, photo ID and referral, if applicable.
- Please bring your health history, including a list of medications and allergies.
- Please bring your glasses and/or contact lenses if you wear these.
- Please be prepared to pay your co-payment, co-insurance or deductible, if applicable, and any past due balances.

Millennium Eye Care participates with most major insurance companies. However, it is very important that you understand your specific insurance policy and adhere to any requirements such as obtaining a referral prior to your appointment. Please call your insurance company for any questions about your policy.
If you need to reschedule your appointment, we would ask for 24 hours notice.
During business hours, patients should call any of our office locations to speak with a staff member. After business hours, our answering service is available to route your call to a member of our on-call patient care team. Please note, any patient with a medical emergency is advised to dial 911 to receive emergency medical care.
